The team who works beside you
Our interdisciplinary hospice team works together to provide thoughtful, compassionate care for patients and ongoing support for families.
Provides clinical guidance and oversight, ensuring each care plan is comfort-focused and centered on effective pain and symptom relief.
Serves as the main point of contact for families. Nurses visit regularly to manage symptoms, answer questions, and adjust care as needs change.
Supports the emotional, social, and practical needs of patients and families, helping guide difficult conversations and connect to helpful resources.
Provides compassionate, hands-on personal care, helping patients remain comfortable, respected, and dignified.
Offers spiritual and emotional support, honoring each person’s beliefs through presence, listening, and prayer when desired.
Continues caring for families after a loss, offering grief support, resources, and follow-up for up to 13 months.
Bring warmth and companionship through simple, meaningful acts of service that create connection and comfort.
